New Patient Information
Our name, Restored Life, says it all-our hope for you is to experience growth and breakthrough while you partner with us. We hope life is restored, so that you can live a full, rewarding, powerful adventure.
As you drive up to our building, you will quickly see that we are not like a typical counseling practice. The peaceful landscape and comfortable atmosphere is designed to help put you at ease and help you begin to relax and feel safe. Feel free to come on inside; no need to ring the doorbell. You will see our large, open, waiting room and our receptionist will greet you and assist you with all your paperwork. She can also help set up your future appointments. Then, feel free to have a seat, grab a cup of coffee and your therapist will come to the waiting room to greet you. All our sessions are about 45-50 minutes in length and we do our best to stay on time.
Please make sure you arrive 30 minutes early to complete paperwork if you are a new client. If you will be more than 15 minutes late, it may be best to reschedule your appointment as this time will be lost due to the fact that other clients will be expecting to start at their scheduled appointment time.
Our friendly and knowledgeable office staff can provide help in choosing a therapist as well as help understanding your copay or payment options and filing insurance for you.
As a courtesy to other clients who are desiring to make an appointment and in order to value our therapists time, please note our cancellation policy. If you will be unable to make your appointment, please contact our office as soon as possible. We require Twenty-four hour notice for the cancellation of an appointment. Appointments canceled with less than 24 hours’ notice will be charged a $100.00 fee. Payment for the missed appointment will be required before another appointment can be scheduled.
Appointments missed due to inclement weather, acute health problems or family crisis will not be charged (this will be at the counselor’s discretion). If you must call after business hours, you are welcome to leave a message with your cancellation. Thank you for your understanding.
